Patient Administration
What is Patient Administration?
The Patient Administration Division (PAD) of Army Health Clinic SOUTHCOM (AHCSC) provides for and coordinates all administrative matters related to registration and outpatient records. Primary services we provide include requesting medical records from other military and civilian medical facilities, copying medical records, conducting Defense Enrollment Eligibility Reporting System (DEERS) checks, initiating line of duty investigations, assisting retirees with transition, and processing requests for release of information requests. All patients 18 years of age must present a military identification card for treatment at AHCSC.
All military family members must be enrolled in DEERS prior to receiving non-emergency medical care. The Electronic Medical Record is the primary record source for patient appointments, however, a copy of outpatient records can be delivered to the clinic as needed. Medical records for all beneficiaries are maintained at the clinic. Questions concerning records may be answered by calling 305-437-2334.

Patient Registration
Once in processing is complete, you must register with the clinic. If you have Medical/ Dental STR’s on hand, they must be turned in. If we do not receive the STR from your previous MTF, we may need you to must come in to fill out a FORM DD887 to request the transfer. Please note, your STR differs from your EHR (Electronic Health Record). EHR do NOT require to be transferred. All Military Training Facilities have access to view your entire EHR. To confirm if you solely have a EHR, please contact the records office.
