Family Member Travel Screening (Overseas Screening)
What is Family Member Travel Screening?
FMTS is a process to screen all family members of active duty and/or activated Service members who request government-sponsored travel to Outside of the Contiguous United States (OCONUS) locations (including Alaska and Hawaii) for potential travel concerns, which may include medical, educational, and/or dental needs, to ensure that the gaining location can meet their needs. The travel concerns are then coordinated between the losing and the gaining FMTS Office prior to executing final personnel actions to the OCONUS location. This screening may result in mandatory enrollment into the Exceptional Family Member Program (EFMP) if the family member(s) meets the EFMP enrollment criteria.
All family members of active duty and/or activated Service members who request government sponsored travel to remote and/or OCONUS locations must be screened. FMTS will be completed within the 12 months prior to the Service member's report date to the projected duty location. The expected timeframe to complete the entire FMTS process is 60 days. Additional screening may be required if the Service member is selected for participation in the Defense Attaché Program. Screening should be initiated immediately once the Service member has been officially notified of an assignment to an OCONUS location.

The MTF will provide physicals to all authorized servicemembers and 18+ dependents, but servicemembers from other services must contact the appropriate point of contact for submitting their packet:

Army Family Member Travel Screening Process
To have a screening completed please e-mail the forms below to our office at usarmy.gordon.medcom-eamc.mbx.southcom-efmp@mail.mil
-One DA form 7246 per family. There is a place on page 2 for a medical provider to sign, please do not worry about having that completed, that will be signed by the medical screener here in our office.
-Your Military Personnel Division (MPD) authenticated DA form 5888 –Part A MUST be signed by the MPD representative. Contact SOUTHCOM/USAG Miami MPD at usarmy.miami.usa.mesg.usag-miami-milpo@mail.mil or 305-437-0739/1936/1717.
Once we have all of the requested documentation that will prompt us to go into the medical records of your dependents to verify they have had a physical exam completed by their provider within the last 12 months. If an exam is not available to review we will contact you to request that one be scheduled and completed before we can finalize the screening. If the latest exam was not done at a Military Treatment Facility, please plan to also e-mail a copy of the civilian medical records along with the above requested documentation.
When the EFMP medical screener has reviewed the medical records she will determine if enrollment into the EFMP is required. They may request additional medical records before they can make their determination, such as a DD form 2792 completed by a treating provider, or a DD form 2792-1 completed by the dependents school, if special services are given in the school setting. The Medical screener will then sign and stamp the DA form 5888, which we will be uploaded into the Web Based EFMP database for the gaining installation to review. The gaining installation will notify HRC when they have approved the command sponsorship request and the Service Member will be notified by their branch manager.
